Anklet of Translocation + Sun School (with a liberal DM). Not much of a pounce though, with just a single extra attack, and a 10 feet teleport range twice per day.

Ardents (and I think Psiwarriors) can get the excellent Dimension Hop power, though. It is a swift action short range teleport, but the range is augmentable, so you can get a bit more distance out of it.

Neither of those include the clause about not being able to act afterwards either. So you could make your normal full attack on top of the extra attack from Sun School.

I believe that this is a goof on WotC's part when transferring info from 3.0 to 3.5 (this is merely speculation on my part, as I do not have a 3.0 PHB/DMG to verify this).

you can find the info in MotP:
- (p.51) a psychic wind
- (p.159) an attack from an Astral Dreadnought
- (p.174) have it cut with a Githyanki Sword

The entries for both the Astral Dreadnought and the Githyanki have the stats for a silver cord:
- character's AC
- hardness 10
- 20 hp
- any time you chord is damaged, you must succeed a DC 13 Fort save or be automatically return to your material body
- if the chord is severed, then both your material body and your astral form are destroyed

I think in this case the grammar is less important than whether the Str and Dex bonus provided to your created undead scales.

Is there an item which always gives you Quick Draw or similar, rather than for a single weapon like Crystal of Returning does? How it does this doesn't matter, I just need to get a weapon into my hand as a free or swift action. I don't need to specifically 'draw' it for Iaijutsu Focus or anything.

Don't forget Totemist 2 with Blink Shirt.  That gives Move-action short-range teleport.
