You have to have permanency on the "hat" to reshrink it, or re-cast Shrink Item on it. So getting it every round would be a bit trickier (you'd need extra standard actions, if you wanted to do anything else). Of course, you shouldn't need it every round...

True, although you could also wear multiple layers of hat. Remember, Shrink Item also can turn whatever you shrink into a cloth-like material.
