I need to know if there is a way to make an undetectable disguise. Something along the lines of poly any object rat into target person + modify memory to implant the memory that the rat was always a person, but was polymorphed into a rat, or some such. Assassins show up to kill said target and instead kill the poly/rat.
Roadblock one, do the assassins get a spot check if they have never physically seen the target? Roadblock 2, does the polymorphed rat remain a polymorphed rat in an anti magic field. RAW it appears to - same kingdom +5, same class (mammal) +2, same or lower intelligence +2, grand total +9, which equals a perment change according to the poly any object rules.
Simpler more elegant solutions are most appreciated. 32k grand total to pull off whatever the scheme is - including all hires and castings.

If the disguise is not impromptu then there is a rakasha spell that does that. I assume this to be the case since you mention PAO. This is really more for the simply Q&A thread though. There is another item that will do what you want.

I believe there's a mask which fools true seeing in Sharn: CoT. Honestly, though it would probably be easier to either modify memory/mind rape the person looking for you to truly believe your death happened. Or else mind rape a doppleganger or changeling into always having been you, and let them get killed.

Depending on the resources you have available, brainwashing an ethereal doppelganger might be even better. Come to think of it, there HAS to be something clever you could do with an ED's Mind Wipe ability.
If an assassin is convinced that the ethereal doppelganger is Target A, and the ethereal doppelganger erases all memory of its existence from the assassin, does the assassin forget that Target A exists?
Or how about this:
1. Your controlled ethereal doppelganger uses Assume Identity to become a perfect duplicate of the target, explicitly including abilities, memories, and personality.
2. You now command the ethereal doppelganger to use mind wipe on itself.
3. Since the ED knows who it is, it erases all knowledge of its true identity from itself--arguably leaving it with ONLY the assumed identity (which is not, strictly speaking, "memory of its existence," but memory of someone ELSE'S existence."
4. Voila: perfect duplicate in all respects.

and if saves are an issue, the controlled aspect can get them to "voluntarily" fail the save.

My favorite combo thusfar is: Changeling +0 LA with Godblooded (Vecna) +1 LA
Simulacrum the shite out of yourself for fun and profit.
Changeling's Minor Shape Change is a physical change and not illusory.
Godblooded (MM5): Vecna-Blooded makes you immune to any divination cast at you AND any cast to learn information about you.
Throw in some Shiftweave (MIC) and some clothing/disguise accessories for haha's.

Problem there is that "No result" is likely to set off alarm klaxons in a canny NPC that something's up. If you're aiming for a target you expect to be divinable, and it's NOT divinable, that's almost as bad as being able to divine that it's not the intended target.
