Faerie Mysteries initiate is ridicolously good for an undead. You can easily get up to something like D12+12 Hp per HD.

Actually head: Wizard/Psion.
Body: Warblade/Factotum.

Warblade body(assume head has higher initiative to start): Head goes, Body goes, Body uses white raven tactics on head, head goes again. Next round, body delays, head goes, then body goes and WRT  .... watch the players boggle as this skeletal figure spouts 2 spells a round. Factotum body. Not as broken... but what level does factotum get extra move actions?

That's actually pretty funny! (considering what it would have to do to get the benefits of that feat) From Dragon 319: "You and a partner with the Faerie Mysteries Inititate feat practice a 15-minute regimen of cultural rituals that grant you special bonuses on skill checks or combat rolls. You choose which bonus you'd prefer before conducting the exercise, modifying your regimen slightly to achieve the desired effect." "Passions: You and your partner engage in an exuberant sensual act. The ritual leaves both partners invigorated, allowing you to use your Intelligence modifier instead of your Constitution modifier to determine bonus hit points." Tantric Skeletons!

Wow, I can't believe how many people missed the Voltron joke.
On Topic: This, is a killer idea. Slap some templates onto the body (or hell, the demi-lich if you want) and go to town. Like most everyone else said, you're free to take pretty much any undead.

Why would they be blind? I don't think "head" is a requirement for a D&D creatures to see...especially undead ones which doesn't have specific organs to begin with (No critical hit/precision based attacks) anyways. -K

undead have darkvision 60 by default.
skeletons dont have eyes by default. eye sockets are not normally useful for sight strictly by themselves.

Specifically: Others, such as golems and undead creatures other than vampires, are not affected by the loss of their heads.
